Interesting to know that these errors still exit. This article points out that the U.S. hospitals have a problem with quality of care in terms of medical errors especially with medication errors. So, the U.S. government as well as the Institute of Medicine, the Joint Commission on Accreditation of Healthcare Organizations and Leapfrog have all recommended the use of Computerized Provider Order Entry (CPOE) system to help prevent errors and instead they are contributing to other errors. CPOE was adopted by hospitals to increase electronic orders and reduce handwritten illegible orders to reduce errors and to sound the alerts for harmful medications when prescribed for more caution and alertness in giving them. It also helps to improve the care processes by giving real time information especially with medication orders or administration and enables sharing of medications, tests, test results, and other pertinent information by employees across the hospitals. CPOE is also used as a form of database for tracking events and their outcomes for analysis (Queenan, Kull, & Devaraj, 2016)

Leapfrog is an organization that surveys hospitals and rates them based on the quality of care they provide and the number of medications errors and alerts they receive. Leapfrog’s CPOE Evaluation Tool was designed by medication safety experts and researchers of which most hospitals are supposed to be using the Leapfrog hospital CPOE evaluation tool to evaluate their CPOE effectiveness. It is also used to improve the clinical decision support of practitioners to reduce episodes of adverse drug events. So, I am surprised that they have not realized that the change of orders and transfer orders are not showing in real time or are not being picked up.
