The factors that can impact cost or revenue in the nursing home facility include:

Antimicrobial Resistance (AMR):

Antimicrobial Resistance (AMR) which is caused by misuse and overconsumption of antimicrobials creates an opportunity for microorganisms including bacteria, viruses, fungi, and parasites to invade and adapt in the host body even in the presence of medication that is supposed to treat the infection (Dadgostar, 2019). AMR poses a significant threat to the global public health systems resulting in uncertainty as the diseases can no longer be treated with antibiotics. Due to the effect of infections with AMR, severe illnesses and complications, prolonged hospital admissions, increase in healthcare costs, higher costs in second-line drugs and treatment failures have been documented. Over two million people in the United States are affected by antibiotic-resistant diseases annually resulting in a minimum of 23,000 deaths and 20 billion dollars in direct healthcare costs in the United States and about 35 billion dollars lost in productivity annually were reported (Dadgostar, 2019). The resultant effect wears down the person’s immune system’s ability to fight infectious diseases complicating the health of most patient populations such as patients undergoing chemotherapy, dialysis, surgery, joint replacement, and people with chronic conditions like diabetes, asthma, and rheumatoid arthritis will be heavily impacted. Patient beliefs and expectations, the lengthy prescription period for antibiotics, and financial incentives all contribute to AMR (Dadgostar, 2019).

Increasing growth of the aged population:

The nursing home/skilled nursing facility houses the aged population due to several reasons such as increasing complexity in health, frailty accompanied by a deficit in self-care and daily activities and loneliness, and need for socialization and poor cognitive ability (Fang et al., 2020). Ageing is accompanied by chronic diseases such as cardiovascular diseases, diabetes, cancers, neurodegenerative diseases especially Alzheimer’s disease (AD) and Parkinson’s disease (PD), digestive diseases, respiratory infections, skin and subcutaneous diseases, neurological diseases, and musculoskeletal disorders. This healthcare challenge imposes a threat in diagnosing and treatment causing great financial and socioeconomic burdens on the organizational level and the policy maker. The majority of this population resides in the nursing home facility at some point as the family’s care discontinues giving way for the healthcare system to take over their care and management (Fang et al., 2020).

Long-term care services:

Long-term care needs are a very essential factor for the aged population. This service provides the individual with assistance with activities of daily living. The needs and services may vary according to individuals to health and physical need. Paying for this service is a major challenge. Approximately $339 billion in spending was reported on long-term care services in 2013 total national spending on these services including about 72% in public spending and Medicaid and Medicare are known to finance these services according to benefit qualification (Harris-Kojetin et al., 209). The skilled nursing home facility can be impacted by these factors with the increasing financial burden of providing quality health care, especially to those without qualifying financial coverage. Awareness is vital to convey an understanding of the causes and complications of over-prescription and overconsumption of antimicrobial agents.
